Payment Information

PAYMENT FOR SERVICE IS DUE WHEN SERVICE IS RENDERED.

Acceptable methods of payment are: cash, check, credit/debit cards through Pay Pal on this website or at our office!
Pay Pal will process: Visa, MasterCard, American Express, Discover and Electronic Checks.
As a courtesy, we will electronically file your insurance for you. Your insurance payments will be sent directly to you from your insurer.
Insurance is an agreement between you and your insurer! You are responsible for knowing your policy and available benefits. If you have questions, we recommend you contact your insurance company for benefit information!

MEDICAID REQUIRES A "Certificate of Medical Necessity" (CMN).

The CMN must pre-date any evaluation and services!
It is your responsibility to obtain a CMN and have your physician complete it. The CMN may be downloaded from this web-page, faxed to you or picked-up from our office.
The CMN must be completed by a physician that is a Medicaid provider.
No evaluations or services shall be scheduled until the CMN is mailed/faxed or hand delivered to beyond therapy pediatric group's office.
The required prior-authorization process is very time consuming and we may ask the parents to assist us in obtaining the necessary documentation and signatures. It is our right to accept a limited number of Medicaid beneficiaries.
