Beyond Therapy Pediatric Group is a Medicaid provider: - All
children require a Certificate of Medical Necessity (CMN),
- The CMN is completed by your child's Doctor or Nurse Practitioner,
- The
CMN must be in our office before your child's evaluation will be scheduled,
- The CMN may be downloaded from this web

Beyond Therapy Pediatric Group is not a Mississippi-CAN provider: - If
your child has been automatically enrolled into the MS-CAN program, you have 90 days to "dis-enroll" and
return to Medicaid.
- The dis-enrollment form must be processed by Medicaid by the 20th of the month for your
child to return to Medicaid on the first day of the following month.
- If you fail to dis-enroll, your child will
be locked into the assigned plan (United Health Care or Magnolia Health Plan) until January 1.
- Medicaid will
repeat this process every year, so please watch for your packet between October and December. Your forms need to be
returned to Medicaid before December 20th of each year. If not, your child will be automatically enrolled in one of
the plans.
